Job Summary and Qualifications
The Seacoast Manager of Case Management is responsible for supporting the Director of Case Management in managing the Case Management operations. This includes providing leadership for planning, management, implementation, integration, and coordination of Case Management in support of facility goals dedicated to excellence, effective leadership, and financial stability. Supporting the delivery of high-quality, cost-effective patient care through activities based on facility strategic goals and objectives.
- Assist the Director with managing clinical operations of the care management team, including hiring and developing Nurse Case Managers, Case Management Assistants, and Social Workers.
- Assist the Director with designing and implementing performance improvement initiatives within scope.
- Assist the Director with yearly employee evaluations and help implement plans of correction when needed.
- Assist the Director in monitoring customer satisfaction scores and implementing plans to improve them.
- Promote and support a collaborative team approach in providing case management services to all patients.
- Collaborate, assess, and make recommendations to the Director to identify opportunities for improvement in patient care management and optimal process and resource utilization.
- Follow through on commitments; accept responsibility for personal actions; and adapt to change.
- Use problem-solving and conflict resolution skills to foster effective work relationships with team members.
EDUCATION REQUIRED
- BSN or on track for obtaining BSN within 2 years of hire date OR an MSW.
LICENSURE/CERTIFICATION
- If RN: Current NH RN license required or appropriate compact licensure.
EXPERIENCE REQUIRED OR PREFERRED:
- 3-5 years Case Management experience preferred.
- ACM or CCM Certification preferred.
SKILLS, KNOWLEDGE, AND ABILITIES:
- Ability to remain calm, to think and act quickly and logically in a crisis situation.
- Excellent verbal and written communication skills.
- Good mathematical skills.
- Requires team player, positive, can-do winning attitude.
- Excels at multi-tasking and managing competing demands in a professional manner.
