The Case Manager 2 provides complex case management duties within the Northern Region medical team in order to facilitate the access and care coordination for an identified caseload of patients through our increasingly complex health system. The Case Manager 2 is an adept collaborator, skilled in managing complex care systems as well as medically and socially complex patients. Creates a work culture of respect, trust and understanding for the highly diverse populations the Health Department serves in the office and in the community.
As a Case Manager 2 you will be responsible for:
Case Management
- Maintain an ongoing caseload to provide complex support for patients navigating multiple healthcare settings.
- Perform complex case management duties, including assessing, evaluating, and documenting eligibility for full spectrum of Federal and State program services and benefits.
- Provide referrals to resources when applicants become ineligible for Medicaid or other programs.
- Conduct a comprehensive needs assessment with consumers, their family, guardian, and collateral contacts to develop an accurate portrayal of client's strengths, needs, choices, and risks.
- Develop service plans with appropriate levels of service and risk mitigation; coordinate care planning, engage in problem resolution, and facilitate referrals with agencies, individuals, and community partners.
- Ensure that the service plan fully addresses assessed care needs such as behavioral support services, home health care, emergency response systems, and other needs, and care plans and arrange services as necessary.
- Maintain an ongoing caseload and track patient progress throughout the identified goal period stated in the care plan.
- Assess and respond to crisis situations involving patients; this includes referral to Crisis Line as appropriate and communicating with internal and community-based services for additional support.
- Interview Medicaid and food stamp applicants, recipient's relatives, guardians, and their representatives to determine and document initial and ongoing eligibility and financial needs in accordance with Federal and State guidelines.
- Provide eligibility redetermination when financial or medical status changes occur; screen for unmet needs such as health care and social services.
Team Based Care
- Provide comprehensive, team-based medical case management to an assigned panel of complex clients in coordination with the medical provider, nurse, and other clinic staff.
- Establish, coordinate, monitor and adjust individual care plans to keep the plans current and relevant to the individual's needs.
- Coordinate care with other systems, arrange for services, advocate for patients, and provide support.
- Assist and consult with clients, family members, medical team, and outside agencies in navigating complex service systems such as mental health, medical, addiction, community resources, and legal services.
- Interpret and educate others on complex medical and psychological terminology and reports.
- Understand and interpret procedures, regulations and legal implications of program services and other various program offerings for clients.
Community Engagement and Outreach
- Maintain relations and general knowledge of local hospitals, available community specialty clinics, behavioral health services, and substance use treatment services.
- Monitor quality of care provided and address problems with coordination or communication in a timely manner, working with other professionals and agencies as appropriate.
- Research and take steps to find new community-based opportunities, meet with external entities to improve partnerships for the benefit of the patient and staff experience, connect and develop relationships with these entities with the purpose of finding shared and conflicting goals.
- Maintain general knowledge of recent developments, current literature and sources of information regarding case management principles, practices and procedures.

Minimum Qualifications/Transferable Skills:
Any combination of experience and training that would likely provide the knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be as noted below. All non-traditional education, training and experience will be evaluated.
Education/Training: Equivalent to an associate's degree; AND
Experience: One (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position. (Equivalency 3-6 years of qualifying training and/or experience).
Other:
- Driver’s License (DMV or like agency).
- BLS CPR Certification.
Skills Needed:
- Effective communication styles that can adjust to the needs of our culturally diverse patient population.
- Compassionate care for socially complex patients.
- Working knowledge of Equity and Social Justice concepts and how to apply them in real settings.
Preferred Qualification:
- Degree in social work or its equivalent.
