Responsibilities:
- Assess older adults and develop a comprehensive plan of service options to meet their needs.
- Interview older adults and caregivers to obtain information about health, functional status, cognitive and psychological functioning, interpersonal relationships, financial and social needs, and home environment.
- Develop and implement a comprehensive plan of care to address the needs and objectives of the patient and family, updating as required.
- Document all assessments, treatment plans, and ongoing monitoring of patients concisely.
- Maintain timely records of all contacts with patients and family members, completing required documents for appropriate billing of services.
- Provide verbal and written communication to other providers involved in the patient's care.
- Coordinate healthcare and all applicable in-home services, acting as a broker in arranging and monitoring services.
- Maintain broad knowledge of community services and appropriately refer patients and their family members.
- Participate in interdisciplinary conferences on behalf of the patient.
- Network, educate, and build relationships with current and potential referral sources.
- Provide "on call" coverage during nonbusiness hours to geriatric care management patients.
Minimum Requirements:
- Graduate of an Accredited School of Nursing (BSN preferred) and WI RN license; OR a Master's Degree in Social Work
- Current WI RN license
- 3-5 years of related experience required.
- Strongly prefer GCM certification and one of the following: CMC, CCM, C-ASWCM, C-SWCM
- Current/Valid WI Drivers License; plus reliable/insured vehicle
Work Schedule: Monday-Friday 8 am-4:30 pm
