Job Summary
The Supervisor of Case Management is responsible for the supervision of the assigned case management team including but not limited to: onboarding of new staff; providing on-the-job training and technical support; monitoring caseloads for accuracy of departmental and system process and policies; State and Federal regulations; identifying areas for process improvement in order to support the case management team; and auditing as directed. Communicates with the Manager and Director as related to overall department, program, mission, goals, budget and performance standards for the area to produce a desired outcome. Provides interventions to assure safe and effective care transitions but not limited to, transitions between inpatient care, rehabilitation facilities, skilled nursing facilities, assisted living facilities, outpatient programs, home health and hospice.
Job Requirements
Experience
Preferred: Minimum of 3 years experience in Case Management
Preferred: Previous Leadership experience
Skills
Represents the department on several process improvement, standardization, and regulatory committees.
Operates strategic plan by setting and monitoring department Key performance measures and goals.
Determines department scope of care or service in relation to values, mission, and strategic plan of the institution.
Assist with developing long term department plans.
Working knowledge in human relations, customer service, inventory management.
Ability to work with multiple people while meeting numerous requests and deadlines.
Goal oriented with initiative and potentials for self-development. Maintains organizational and priority setting skills.
Knowledge of CMSA’s Case Management standards of practice.
Knowledgeable concerning Inpatient/Subacute/Acute rehabilitation regulatory standards.
Licensure/Certification/Registration
Preferred: Current Licensure in respective discipline.
