The Registered Nurse (RN) – Case Manager, Seniors Mental Health (SMH) Outreach, will work as part of an inter-professional specialty SMH Outreach team providing comprehensive assessment, consultation, risk management, and support for older adults living in the community with complex cognitive-mental health needs, often with multiple physical co-morbidities. The RN Case Manager will assist with the development of an integrated delivery of service to enhance the quality of life for older adults with complex cognitive-mental health needs and/or behavioral disturbance through partnerships with various community services supporting seniors across the system of care. The RN Case Manager will coordinate and integrate services and resources to assist the older adult with aging at home and promote system integration. As a member of the SMH Outreach team, the RN Case Manager will practice in the role of Case Manager utilizing the knowledge, skills, and abilities of a full scope Registered Nurse in order to provide specialty service to the target population and also to enhance, augment and develop resources in the community.
Skills and Qualifications Required:
- Bachelor’s degree in Nursing, preferred or an equivalent combination of education and experience
- Current unrestricted registration with the College of Nurses of Ontario
- Education completed or experience working in the PIECES framework
- Minimum of 2 years of recent experience in geriatric psychiatry and/or geriatrics
- Minimum 1 year of experience working in a community nursing setting, 3 years preferred. May consider relevant experience in LTC.
- Demonstrated recent experience in working with families and community support services, preferred
- Previous experience as a case manager preferred
- Experience in assessing educational needs and/or developing educational programs for colleagues, service providers, clients and family members preferred
- Gerontological Nursing certification through Canadian Nurses Association; OR, Gerontology Diploma; OR, Psychiatric/Mental Health Nursing certification, preferred
- Community development experience, preferred
Position Responsibilities:
- Works independently to provide comprehensive assessment, consultation, and treatment recommendations for clients residing in their home, hospital, LTCH, or Retirement Home, along with follow-up and discharge planning once symptom stabilization has occurred
- Obtains and documents an initial database of information, from all appropriate sources of information, as part of their Initial Assessment including a plan for evaluation and further assessment
- Provides triage including assessment, prioritization, system navigation, and facilitates consultation with other interprofessional SMH Outreach team members
- Collaborates with other members of the SMH Behavioural Support Services (BSS) team as required including the BSO Mobile Response Team, and Psychogeriatric Resource Consultant
- Collaborates with family, care partners, and professional care providers, to develop practical and effective strategies to support responsive behaviours in dementia to reduce risk in community and long term care settings
- Responds to LTCH, emergency departments and acute care services, in a timely manner
- Provides home visits, assessments and service plans; reviews and evaluates the treatment and ongoing care plan in consultation with the family, care partner/ care provider
- Develops and sustains partnerships with other service agencies within the community to ensure a coordinated and seamless approach to service delivery
- Utilizes knowledge of community services and resources, and relationships with these partners, to facilitate appropriate linkages as required.
