Position Summary
This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities.
The RN, Clinical II, Outpatient provides intermittent visits to terminally ill and palliative patients and families, providing symptom management and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing, and psychosocial support to provide appropriate care as the patient transitions to the end of life.
Essential Functions and Responsibilities of the Job
- Utilizes the nursing process and practice standards as a framework for the management of patient care.
- Analyzes the assessment data to determine a nursing diagnosis and appropriate interventions.
- Demonstrates leadership in the coordination of patient care across the healthcare continuum with progress towards meeting desired patient outcomes.
- Demonstrates the ability to utilize effective critical thinking skills.
- Prioritizes essential functions.
- Demonstrates knowledge of the clinical information system and applies principles to patient documentation.
- Contributes to an effective and safe working environment.
- Formulates a teaching plan, based on identified patient learning needs, and evaluates effectiveness of learning; family is included in teaching plan as necessary.
- Demonstrates knowledge and understanding of patient privacy rights. Maintains confidentiality of all medical, financial, and other sensitive material in printed, electronic, or verbal form, which may jeopardize the privacy of patients and or employees.
- Demonstrates the ability to use good customer service skills by managing and acknowledging patient-family-significant other expectations.

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
Minimum Requirements
Qualifications/Work Experience:
- 1-2 years relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred.
- Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings.
- EMR experience preferred.
- Must have 2 days available to pick up shifts from Monday to Friday and available for at least one shift on the weekend monthly.
Education/Licensure/Certification:
- Current RN license issued by the California Board of Registered Nursing.
- BSN preferred.
- BLS issued by the American Heart Association.
- Hospice and Palliative Care Certification preferred.
