LVN, Licensed Vocational Nurse (Per Diem)
Join our leading home health services team in California as a Licensed Vocational Nurse (LVN), where you will provide professional nursing care to patients in their homes. This Per Diem role allows you to work flexibly in a supportive environment, ensuring high-quality care and patient satisfaction under the guidelines of the Nurse Practice Act in the State of California. You will work collaboratively with other healthcare team members to deliver excellent medical treatment and attention.

Essential Duties and Responsibilities:
- Provide skilled nursing care within the scope of LVN practice, including administering medications, changing bandages, and collecting specimens.
- Gather data pertinent to the patient's health needs and develop a care plan.
- Assist Registered Nurses (RN) and doctors in educating patients and their families about the treatments and care plans.
- Offer primary medical care and support in daily living activities to enhance patients' health, comfort, and safety.
- Monitor and record changes in the patient's condition, and report these to the supervising Nurse Practitioner or Physician.
- Facilitate home health visits and Follow-up visits, ensuring adherence to each patient’s specific healthcare requirements.
Minimum Position Qualifications:
- At least one year of clinical experience in nursing.
- Active LVN license in California.
- Current driver's license with a clean DMV record.
- Ability to travel to patients' homes within designated regions.
- Demonstrated proficiency in clinical skills and nursing care.
Preferred Qualifications:
- Previous experience in home health services.
- Advanced certifications relevant to home health care or geriatric care.
- Excellent communication and interpersonal skills to interact effectively with patients and families.
- Strong organizational and time management skills to handle the caseload efficiently.
- Cultural competency to work with diverse populations.
