DESCRIPTION OF POSITION:
- The Clinical Case Management RN Case Manager assesses the physical, functional, social, psychological, environment and financial needs of clients/patients; identify a cost -effective comprehensive plan to meet the families' service needs, and implement the plan.
SUPERVISION: Reports directly to Director of Clinical Case Management
RESPONSIBILITIES AND DUTIES:
- Provide referrals to appropriate community resources.
- Facilitate access and communication when multiple services are involved.
- Monitor activities to ensure that services are actually delivered and meet the needs of clients.
- Coordinate services to avoid duplication.
- Assess the clients' formal and informal support systems.
- Monitor clients progress toward goal achievement and periodically reassess changes in health status.
- Monitor the care plan to ensure the effectiveness and appropriateness of services.
- Act as patient advocate; identify and develop new community resources; assist with problem solving.
- Communicate clinical information to payers to authorize days for billing.

EDUCATION AND WORK EXPERIENCE
- Graduate of an approved School of Nursing, BSN preferred, Case Management certification preferred.
- Minimum of three years med/surg experience in an acute care hospital.
- Computer skills at data entry level.
- Current Louisiana RN License or Compact State License and CPR certified.
- Knowledge of community resources and programs.
Physical Demands/Work Environment
- Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time.
- Must be able to respond quickly and effectively to emergency and non-emergent situations.
- May be required to assist in controlling disorderly conduct or combative patients.
- Must be able to exchange accurate information with patient, family, peers and medical personnel.
