Description
The Social Work Care Manager provides Social Work Services in accordance with policy and procedures of St Patrick Hospital and Health Sciences Center in collaboration with the patient and patient's family. Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community.

Shift details: Full-time, 1 FTE (40 working hours), day shift (Schedule: Monday to Friday, 8am to 4:30pm)
Required Qualifications:
- Bachelor's Degree - Social Work, Psychology, Sociology, or health related field.
Preferred Qualifications:
- 2 years - Experience in Medical Social Work
