Job Description:
- Reaches out to the PCP to inform of discharging patients via fax.
- Reaches out to the PCP to obtain orders.
- Ensures all due admissions (all new admissions) are completed daily and staffed appropriately.
- Ensures recertifications are completed daily, deciding at the end of the episode if a recert is appropriate, ensuring recerts are staffed by your assistant.
- Answers/responds to all emails.
- Requests authorizations.
- Knowledgeable of DSL and where to find patient information, PCP, orders.
- Ensures labs are completed and results are unloaded in the chart and reported to the MD who ordered them as well as the PCP and pharmacy, if necessary.
- Schedules/plots/TAC/reason in notes, visits as needed if assistant is not available.
- Ensures all lab forms are correctly filled out.
- CM to verify from the hospitals and update QA.
- Ensures completion of ROC's that they are staffed out and orders are updated into the chart.
- Writes communication notes of any changes and makes notes on all phone calls.
- Ensures CHHA care plans, fall reports, incident reports are tasked and completed as needed.
- Creates orders for other clinicians.
- Calls PCP when necessary for change of condition.
- Understands workflow process.
- Well-versed in all insurances.
- (Regal cases) A report to be submitted twice a week.
- Attends case managers meeting.
- Anything else requested by their managers.
Requirements: Must be a registered LVN or have more than 2 years of experience in this field.
