Job Overview
We are seeking a dedicated and skilled Physical Therapist to join a reputable home health team in Joshua Tree, California. This role focuses on delivering high-quality, patient-centered therapy services to individuals in their homes. Key Responsibilities
- Patient Care: Conduct evaluations of referred patients, create and implement personalized treatment plans, and provide direct physical therapy services in alignment with each patient's goals.
- Communication: Engage with patients and their caregivers about progress, challenges, and ongoing home therapy activities. Collaborate effectively with an interdisciplinary team to ensure cohesive care.
- Documentation: Maintain thorough records, including evaluations, treatment plans, and progress notes, according to professional and departmental standards.
- Professional Development: Stay current on therapy practices and procedures through continuing education, workshops, and professional development opportunities.
- Quality and Safety: Follow all safety guidelines, infection control protocols, and established departmental policies to ensure high-quality care.
Requirements
- Education: Bachelor’s Degree in Physical Therapy.
- Experience: At least 2 years of physical therapy experience, with a preference for candidates who have home health experience.
- Licensure/Certification:
- Active California Physical Therapy License.
- CPR Certification.
- Valid CA Driver’s License.
- Continuous automobile insurance coverage.
- Physical Demands: Ability to stand/walk for prolonged periods and assist in lifting, positioning, and transferring patients and equipment.
Additional Information
- Must comply with mandatory vaccination requirements, or submit for religious/medical exemption.
- The position involves frequent travel to patient homes; reliable transportation and willingness to travel are required.
