Shift: Days
Hours: 08:30am - 5:00pm
Schedule: Monday thru Friday
Primary Duties
- Maintain established departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
- Evaluate referred patients and develop treatment plans including specific goals and activities, reevaluating and modifying plans as necessary.
- Provide instructions in therapeutic procedures to be continued by the patient.
- Provide direct physical therapy to patients per plan of care.
- Maintain active and supportive communication with the patient/caregiver regarding progress, problems, home programs, and other issues related to the therapeutic process.
- Prepare written documentation as required by the profession and the department such as evaluation results, individualized treatment plans, and progress reports.
- Communicate observations, facts, and comments with the interdisciplinary team.
- Attend patient care conferences and other related meetings.
- Assist in the Quality Improvement Program and in providing in-services to other staff.
- Enhance professional growth and development through participation in education programs, current literature, workshops, and in-service meetings.
- Demonstrate sensitivity to the needs of all cultural and age groups, predominantly the geriatric and dying patient.
Position Qualifications
Minimum Education:
• Bachelor’s Degree in Physical Therapy
Minimum Experience:
• 2 years of experience in physical therapy
Required Certification/Registration:
• CPR Certification
• California Physical Therapy License
• Valid CA Driver’s license
• Continuous automobile insurance coverage
Preferred Courses/Training:
• Home Health Experience
Physical Demands:
• Prolonged, extensive, or considerable standing/walking
• Lifts, positions, pushes, and/or transfers patients
• Lifts supplies/equipment
