FULL TIME/ 40 HOURS/ CHARLESTOWN, MA / CLINICAL SOCIAL WORKER LICSW
- HYBRID work schedule including onsite and virtual/remote.
- Seeking LICSW (May consider someone in process for independent license).

JOB HIGHLIGHTS
- Provides clinical services with adult patients, couples, and/or families as part of a behavioral health team providing integrated care in a community health center.
- Provides state of the Art Therapy dedicated to enhancing the emotional, social, and physical well-being of patients by employing patient centered, patient directed approach to care.
- Conducts psychosocial assessments, collaborates to build treatment plans, and provides indicated therapy, utilizing evidence based and culturally sensitive models of care.
- Collaborates with colleagues in Internal Medicine and other health center professionals in developing and implementing care plans.
- Provides social work consultation to other disciplines within the Health Center and community.
- Facilitates clinical education of social work students.
- Participates on Departmental, Health Center, and Hospital committees.
- Considers the appropriate and efficient use of Health Center resources and supports implementation of innovations in care delivery as appropriate.
Clinical Practice
- Provides psychosocial assessments of patients and families. Formulates diagnosis, disposition, and treatment plans. Employs clinical services such as individual, couples, family, and group therapy; provides crisis intervention; participates in case management and discharge planning.
- Provides treatment that is evidence based, incorporating updated clinical care models. Develops effective treatment plans and modifies based on patient progress/outcomes.
- Provides care that is culturally/racially sensitive, and trauma informed. Advocates on behalf of patients and/or their families to gain access to services and resources.
- Participates as a key member of integrated medical and behavioral health care teams, participating in treatment planning and providing outpatient therapy.
- Refers to and collaborates with interdepartmental specialists and programs, outside agencies, facilities, payors, and other relevant community resources.
Organizational/Administrative Skills
- Takes responsibility for own administrative duties, including timely and appropriate documentation in patient medical records. Effectively utilizes electronic medical record system to maintain documentation and communicate with other team members.
- Monitors and manages productivity independently to established standards.
- Attends and participates in team meetings, unit meetings, division meetings, and staff meetings.
Professional Conduct
- Adheres to the profession’s clinical standards and code of ethics. Adheres to the Department, Health Center and Hospital standards, policies and procedures.
- Maintains the standard and educational requirements specific to the position and professional licensure.
- Responsible for own professional development. Identifies own learning needs and goals. Attends pertinent continuing education and professional activities related to job responsibilities and career goals.
QUALIFICATIONS
- Master's of Social Work (MSW) from an accredited program required.
- Massachusetts Licensed Independent Clinical Social Worker (LICSW) required.
- Bilingual in Spanish preferred but not required.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED
- Strong assessment and treatment skills, utilizing evidence-based approaches and protocols.
- Excellent organizational skills.
- Strong time management skills; ability to re-prioritize tasks as needed to meet demands with little supervision.
- Demonstrated ability to communicate effectively orally and in writing.
- Demonstrated ability to navigate electronic health records.
- Strong interpersonal skills enabling effective team collaboration.
- Demonstrated ability to understand the role of social worker in a community healthcare setting and to work in a complex, fast paced environment.
WORKING CONDITIONS:
- Able to provide assessment services in office or remotely using virtual technology.
- Able to communicate orally and in writing.
- Able to document services provided using electronic medical record technology.
