Position Summary
The Community Health Worker (CHW) serves as a bridge between the patient and Access Community Health Network and other medical systems. The CHW works to build trusting relationships with community members helping to connect them with the quality care needed. This is a hybrid, grant funded position.
Core Job Responsibilities
- Coordinate and provide health education and disease management education.
- Help individuals navigate the health and social care system.
- With supervision, provide case management services.
- Conduct community outreach activities including health fairs, community events, group sessions (virtual or in-person).
- Identify barriers to accessing quality care. Work with individuals to overcome the barriers and provide referrals.
- Conduct home visits for certain interventions as required.
- Perform other duties as assigned.
Requirements
- High school diploma or GED required.
- Previous experience in providing health education preferred.
- Knowledge of Chicago’s West Side and/or South Side communities (resident of West Side or South Side communities preferred).
- Beginning proficiency with Microsoft Office Suite including Word, Excel, and PowerPoint.
Working Conditions/Equipment
- Working conditions vary, including indoors (health centers, hospitals, community sites, patient homes), and outdoors (health promotion events).
- Remote working options; must have a suitable and secure remote working environment.
- Local travel between health centers, hospitals and home/community visits is required; mode of transportation required, if personal vehicle is employed, a current driver’s license and proof of insurance is required.
- Ability to sit for up to 4 hours at a time.
- Ability to stand for up to 2 hours at a time.
