POSITION SUMMARY:
The Clinical Intake Professional will serve as an integral part of a clinically focused team that provides older adults, their families, and caregivers with a single point of entry and seamless connection to CJE SeniorLife's social service programs, as well as providing relevant guidance and referrals to external resources. The Clinical Intake Professional works closely with the CJE Concierge front-line staff who serve as the first point of contact for new and returning clients, families, and professionals, and assists both Concierge and Intake roles in triaging, scheduling, and direct client service via intake consultations for new and returning agency clients. The Professional also assists in and monitors the status of client intra-agency referrals across the agency's multiple Community Services teams and programs, and provides a connection and warm handoff to services in line with agency policies, procedures, and best practices to serve older adults and families in a person-centered, trauma-informed manner. Successful candidates are adept at screening, triaging, assessing, and connecting clients to appropriate internal and external service providers, as well as monitoring and measuring outcomes. The Professional is cross-trained to assist and support the CJE Concierge role as well as general functions of the Clinical Intake team. Candidates must be comfortable learning and using various technology platforms as well as be at ease handling emergent and urgent issues and triaging prospective clients in occasional crisis situations. This position may maintain a reduced short-term client caseload with commensurate administrative responsibilities. This is primarily an office-based hybrid position, working both from our headquarters at the Bernard Horwich JCC in West Ridge/West Rogers Park, Chicago, and remotely.
Requirements:
- Master's Degree in Social Work, Professional Counseling, or Marriage and Family Therapy. Licensure preferred.
- Experience screening, triaging, assessing, and connecting clients to appropriate internal agency and external service providers, as well as monitoring and measuring outcomes.
- Exceptional interpersonal and client service skills and aptitudes, and a calm, patient presence in all interactions with clients, colleagues, and peer professionals alike.

POSITION DUTIES, FUNCTIONS, AND STANDARDS:
Essential duties and responsibilities include the following (other duties may be assigned):
- Work closely with both CJE Concierge and Clinical Intake teams to screen, triage, and document new and returning client inquiries, as well as internal referrals from agency colleagues, via live call, email, and voicemail.
- Schedule and facilitate brief biopsychosocial assessment with clients via phone, video, or in-person, and help match desired services with appropriate staff and programs.
- Assist clients and colleagues by monitoring and tracking client status of referrals, agency program information, and program capacity/availability, and help maintain waitlists while referring prospective clients to appropriate service providers possible when the client is best served outside of CJE SeniorLife.
- Actively serve on a team of client-facing clinical staff offering occasionally needed rapid response via in-person and phone-based support to clients during business hours, e.g., unscheduled urgent client visits, clients in distress, or other time-sensitive client matters.
- Utilize a detailed understanding of both CJE Concierge and Clinical Intake Team duties via cross-training to assist, support, and provide occasional back-up for the CJE Concierge role as well as general functions of the Clinical Intake team.
- Gather and document personal, financial, health, and insurance data as appropriate during intake calls, advising clients of agency processes, expectations, and client responsibilities.
- Accurately document referral source, intake information, and relevant clinical data in congruence with documentation standards.
- Convey relevant intake and contextual information to appropriate parties within receiving programs at CJE SeniorLife; confirm reception of client to appropriate programs and ensure warm handoff.
- Perform basic online research and networking with internal and external colleagues and peer professionals to uncover, organize, and maintain relevant social service-related resources and information to benefit CJE clients and Community Services staff.
- Uphold policies and procedures as stated in agency manuals and the clinical standards as related to the front end of the client journey.
- Understand and represent the specializations of each practice area of CJE SeniorLife, including whom they serve and how, limits of scope, and recommending specific external resources should CJE SeniorLife be unable or not appropriate to serve a prospective client.

POSITION QUALIFICATIONS:

- Undergraduate or master's degree in social work, Professional Counseling, or Marriage and Family Therapy. Licensure preferred.
- Preferred: Licensure and master's degree, and a minimum of 2 years of relevant work experience in a clinical social service setting.
- Tech-savvy, and comfortable using CRM software, Zoom, Microsoft Office suite of apps.
