One of our top clients is seeking a dedicated and patient-centered RN Case Manager to join our team. In this role, you will play a vital part in ensuring our residents receive the highest quality of care and experience a fulfilling lifestyle.
About You:
- A registered nurse (RN) with a current Oregon license.
- Minimum 1 year of experience in long-term care or a related field (preferred).
- Strong assessment and care coordination skills.
- Excellent communication and collaboration skills with residents, families, and healthcare professionals.
- Compassionate and patient-centered approach to care.
- Proficient in documentation and medical record keeping.
- Ability to work independently and as part of a team.
What You Will Do:
- Conduct comprehensive resident assessments to develop individualized care plans.
- Coordinate care with physicians, therapists, social workers, and other healthcare providers.
- Educate and empower residents and families regarding their care options.
- Monitor resident health and well-being, reporting any changes to the healthcare team.
- Advocate for residents' rights and ensure their quality of life.
- Maintain accurate and up-to-date resident records.
- Participate in interdisciplinary team meetings and collaborate to achieve resident goals.
