Nurse Navigator ATRMC (Utilization Management)

Position Summary
The Nurse Navigator coordinates diagnostic and treatment processes and follows up at assigned Asante facility(s). In this role, the Navigator guides patients and their families through the continuum of care and collaborative process with medical care providers and ancillary support programs; acts as a central point of contact for all medical care providers working with the patient, and facilitates evidence-based nursing care; assesses educational needs and designs, teaches and/or coordinates educational programs for patients, families, and the community; may promote services of Asante's Women's Imaging Centers and may provide oncology education, follow up, and assistance for other oncology patients as needed, if applicable; and may screen inpatient heart failure and high-risk admissions for readmission risk, if applicable.
Qualifications
Education
- Bachelor of Science in Nursing, or equivalent combination of education and experience, is required
- Master's Degree in Nursing, preferred
Experience
- Minimum 3 years of clinical nursing experience, including bedside patient care, is required
Licensure/Certifications
- RN: Registered Nurse licensed by Oregon State Board of Nursing is required upon start
- BLS: Basic Life Support for the Healthcare Provider certified by the American Heart Association (AHA) is required upon start
