Your Role on the TeamPosition Summary: Under the general supervision of the Senior Pharmacy Manager, incumbent performs duties on a broad scope which require extensive experience in Ambulatory Care and application of knowledge of the total pharmacy service. Must be self-motivated and geared to produce outcome-driven results. Establish pharmacy services in assigned specialty clinics and work in a multidisciplinary team to provide optimal patient care. Duties will include, but not limited to, provision of patient education on medication regimens, ensuring adherence, performing clinical monitoring, as appropriate, making drug-related recommendations/interventions based on clinical judgment, and updating patient profiles and other forms of clinical documentation. This pharmacist may participate in additional clinical and operational projects as assigned by the supervisor. There may be occasional dispensing roles in the outpatient/specialty pharmacy. This pharmacist is responsible for the technical duties of the specialty pharmacy, including proper documentation and maintenance of records and data, quality control/assurance and performance improvement, inventory control, delegation of assignments amongst staff, and compliance with pertinent hospital policies and State regulations. This pharmacist may also be assigned to precept pharmacy residents, students, technicians, or interns as required by the supervisor. This position is a Direct Care Provider as defined in Pharmacy Policy.

Required Qualifications:
- Three years of clinical experience as a licensed pharmacist in an Ambulatory Care setting.
- Possession of the legal requirements for the practice of pharmacy as determined by the CA Board of Pharmacy
- Must demonstrate customer service skills appropriate to the job
- Knowledge and abilities necessary for the successful performance of the duties assigned to the position
- Graduation from an accredited school of pharmacy
- Excellent planning, organization and time management skills
- Current BLS or ACLS certification
- Ability to establish and maintain effective working relationships across the Health System
Preferred Qualifications:
- Pharm.D. highly desirable
- Hospital pharmacy residency
