Major Responsibilities:
- Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems.
- Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient's size, organ functions, and disease states.
- Facilitates medication-related continuity of care at admission, transfer and discharge.
- Reviews and accurately interprets medication orders.
- Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling.
- Practices in compliance with department policies & procedures, as well as local, state and federal regulations.
- Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice.
- Actively supports procedures and initiatives to prevent medication events.
- Reports events through appropriate channels.
- Actively supports department medication safety and quality improvement activities.
- Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training.
- Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
Licensure, Registration, and/or Certification Required:
- Pharmacist license issued by the state in which the team member practices.
Education Required:
- Bachelor's Degree in Pharmacy.
Experience Required:
- Hospital pharmacy experience preferred.

- Maintains skills in areas of practice.
- Strong computer skills.
- Excellent communication and organization skills.
- Excellent presentation, facilitation and training development skills.
- Demonstrated decision making, team building, customer service, and conflict resolution skills.
- Life support training courses may be required dependent on department discretion
Physical Requirements and Working Conditions:
- Must be able to stand for long periods of time.
- Will occasionally lift up to 35 lbs. without assistance.
- Must have functional speech, vision, and hearing.
- Will be exposed to chemical hazards.
- Operates all equipment necessary to perform the job.
- Exposed to normal office environment.
