Clinical Pharmacist II

The Hope Infusions Clinical Pharmacist II functions at the highest level of clinical practice and works as part of the multidisciplinary health care team to directly care for Hope Infusions patients. Takes ownership of workflows and processes and helps design, implement, document, and monitor medication management and therapeutic care plans through direct interactions with patients and providers, both internal and external, to achieve positive patient-centric outcomes. The clinical pharmacist is a liaison between Pharmacy Services, Hope Infusions providers, outside referring physicians, nursing, front desk staff, and financial clearance teams to support care coordination to achieve positive patient-centric outcomes.
As a successful candidate, you will:
- Provide medication management services, coordinating all clinical pharmacy functions for Hope Infusion patients to help achieve positive patient-centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams.
- Complete patient profile reviews and participate in huddles and meetings as an integrated member of a multidisciplinary team.
- Review medication regimens for appropriateness, contraindications, and potential interactions.
- Help ensure the medication dose, frequency, etc. are clinically appropriate per the referring provider's therapy plan and will support the monitoring of medication treatments.
- Review and provide all laboratory test results to the referring provider as well as other diagnostic studies necessary to support a patient's medication therapy regimen.
- Collaborate with pharmacy leaders, leadership, and staff to develop policies, procedures, and processes for more efficient processing within the Hope Infusions program.
- Work with multidisciplinary teams to develop, implement, evaluate, and update EHR order sets, Treatment and Supportive Plans to promote best practice care.
- Serve as a liaison between Pharmacy Services, Hope Infusions providers, outside referring providers, nursing, and front desk staff.
- Actively participate in organizational, regional, and national patient safety initiatives as determined by COH strategic goals and mission.
- Perform all other duties as assigned by pharmacy leadership.
Qualifications
Your qualifications should include:
- Graduate of ACPE-accredited Pharmacy School or School with ACPE accreditation pending.
- PGY1 and PGY2 residencies, or PGY1 plus two years of experience in ambulatory or inpatient infusion, or five years of experience in ambulatory or inpatient infusion.
- Current CA Pharmacist License.
- BLS certification through American Heart Association.
