We are looking for a Care Navigator (Program Coordination) to join our Payer team. In this role, you will be responsible for providing program and initiative support for our new PMO in our Payer business unit. You will coordinate with various teams, including medical directors and clinician reviewers, acting as a link between different functional areas. Your contributions will support the seamless transition of patients from acute care to a post-acute care setting. This role partners with stakeholders across WellSky to ensure program deliverables are executed.
A day in the life!
You will be responsible for the following:
- Provide coordination and support for various programs and initiatives, including assisting with development and administration of programs.
- Support program requirements at multiple stages through successful completion, meeting all deadlines, executing project-related tasks, and accomplishing defined deliverables.
- Maintain accurate documentation, collateral, and reporting to support program operations and goals.
- Troubleshoot and research best practices to provide process improvement and issue resolution.
- Provide timely and ongoing communication to appropriate stakeholders.
- Perform other job duties as assigned.

- Bachelor's degree in a related field.
- 0-2 years related work experience.
