Team-Based Case Manager – Marietta Region

CareStar, Inc. is currently seeking a Community-Based Case Manager in the Clinical Department to assist and enable individuals/participants to remain in the least restrictive environment, while maintaining the greatest amount of independence and human dignity; assists the participant with access to and coordination of services to enable the individual to receive a range of appropriate services in a planned, coordinated, efficient and effective manner; promotes and supports the participant’s preferences, values, and the right to self-determination.
THE POSITION
CareStar is looking for motivated individuals who:
- Maintain knowledge of Medicare, third-party payers and community resources and program specific resources, Americans with Disabilities Act and State administered waivers; take appropriate action to keep up to date with current practices including attending meetings and training sessions as required.
- Maintain knowledge of the program and Medicaid Rules.
- Perform caseload coverage by fulfilling Case Manager duties as needed and by providing coverage reports, if appropriate.
- Assist the clinical team with special projects as assigned by the Clinical Supervisor.
- Function as the team leader in the development, implementation and coordination of the individual’s care plan, including the authorization of the amount, scope and duration of services, while meeting all qualifications and standards as specified by program guidelines; maintain ongoing contact with paid and unpaid services providers and/or caregivers.
- Complete home visits and/or appropriate contacts, in compliance with the Case Management Guides.
- Have an active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse.
- Have 12 months experience in home and community based services within the past 5 years (preferred).
- Have knowledge of Medicare, Medicaid, home health care and Ohio Home Care waiver programs (desirable).
- Adhere to the CareStar Rule in performance of job responsibilities.
- Understand and comply with CareStar Policies and Procedures.
- Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
