Performs concurrent, extended stay and retrospective reviews of admissions using established criteria to determine appropriateness of admission and extended stay. Serves as a resource to medical and hospital staff on utilization and regulatory issues and participates in coordination of individualized patient care services. Collaborates with other members of the healthcare team to monitor and evaluate patient status focusing on intermediate goals and expected outcomes. Explores and implements strategies to reduce resource consumption and length of stay for the case managed patient population. Issues admission denial letters. Appeals letters are written in response to potential quality and/or admission denials. Facilitates appropriate after care activities by initiating and assisting in ongoing discharge planning activities, making appropriate referrals and maximizing the continuum of care for patients assigned to his/her assigned unit. Participates in special projects as identified in conjunction with a multidisciplinary group examining care processes. Identifies over/under utilization of resources. Has the unique ability to follow the patient from admission to discharge and provide the physician and nursing staff with the best possible options for post-hospital care for the patient.
Minimum Requirements:
- Graduate of an accredited School of Nursing.
- Three (3) years in an acute care setting.
- Current US state RN license with authorization to practice in the state of GA.
Utilizes a variety of standard office equipment to include computer, printer, fax machine, copier, telephone, etc. Basic knowledge of statistics encouraged.
Successful candidates will be subject to criminal background and drug screens.
