Job Summary:
In partnership with the L. Minor Care Administrator and the Care Center team, the Care Manager assists in providing a safe community for guests with special needs, high barriers and vulnerabilities due to mental and behavioral health, physical disabilities or age. As part of a multidisciplinary team, this role intentionally collaborates to improve the guest’s quality of life while coordinating appropriate community placement and housing solutions.
Duties/Responsibilities:
- Intentionally seek to exemplify Jesus Christ daily and be prepared to share the gospel as opportunities arise.
- Assist the administrator in the daily activities and group sessions, guest coordination and case management, strategic planning and solution-focused guidance to ensure unity within the purpose and goals of the Mission.
- Be the advocate for the guest in restorative discipline administrator review meetings.
- Meet regularly with guests with special needs to create comprehensive, strength-based goals and to coordinate services and assistance with community partners in the area of primary care and behavioral appointments, transportation, caregiving, supportive living and wellness planning.
- Proactively provide guests with clothing and anticipate the needs of new guests by collecting their clothing size and needs.
- Report all incidents and maintain case and case notes. Provide appropriate verbal and documented observations of guests to the administrator.
- Be prepared to facilitate small group classes weekly focused on the Gospel, spiritual wellness and self care, leisure activities and healthy habits for the guests. Conduct morning focus meetings each weekday with guests to review daily schedules, encouraging messages and care management meeting times.
- Respond appropriately to emergency situations and be prepared to deescalate emotional and spiritual crisis situations.
- Provide additional support and care to guests one evening a week.
- Other duties as assigned within the scope of this position.

- Have a desire to serve the Lord through widely varied responsibilities to advance ministry to those currently homeless, and the urban poor of Kansas City, with a compassionate heart.
- Agree to City Union Mission’s Statement of Ministry:
o Be a genuine believer in the Lord Jesus Christ, as expressed by personal testimony and conduct.
o Be a member in good standing of a recognized protestant, evangelical church for one year or more.
o Have a genuine concern for the ministry of City Union Mission.
o Have a minimum of one year of sobriety from alcohol and drugs and six months from tobacco.
o Agree to abstain from alcoholic beverages, illegal drugs, and tobacco while employed by City Union Mission.
o Have a lifestyle that is free of sexual sin as clearly defined in the Bible.
Required Skills/Abilities:
- Must have the spiritual maturity and ability to interact with various types of people and have a sound understanding of mental health, disabilities and/or age related challenges.
- Must be able to provide men discipleship, direct care, transportation and support to the same gender and provide caregiving services as needed.
- Demonstrated experience exhibiting compassion, competence in calling to serve the underserved and the capacity to collaborate with a team.
Education and Experience:
- Required: High school diploma or equivalent
- Required: 2 years of related experience in residential group settings, veteran housing, care facilities or case management of vulnerable populations.
- Preferred: 2 year education in related fields, Peer Specialist training, Servant Leadership Internship or similar program completion.
Physical Requirements:
- Ability to ascend/descend stairs and conduct center rounds multiple times throughout the shift.
- Must be able to lift 15 pounds at times.
- Ability to assist guests to exit in times of emergency evacuation.
