We need a Physical Therapist for a traveler position in Iowa! This role involves working in both Estherville, Iowa (50%) and Jackson, Minnesota (50%) SNF and Home Health. Candidates must have both Minnesota and Iowa licenses, but can start with one active license as Iowa is compact.
Additional details:
- Occasional Windom SNF coverage
- Drive Time and Mileage paid between facilities and during Home Health travel
- 1 PTA to supervise
- Software - Netsmart for Home Health and Net Health for SNF - training available
- Offering at least 13 weeks with an option for longer-term (up to 1 year)
- Flexibility in schedule for time off requested or to shift worked days: Normal is M-F, but could do Su-Th to have F off
- Home Health is approximately 6 clients per day with SNF productivity expectations of 85% (includes evaluations and group/concurrent time in full)
- Guaranteed - 36 hours
