JOB SUMMARY:
To provide Hospice care to terminally ill patients twenty-four hours a day, seven days a week. Works as a member of the Hospice team in providing care and oversees all Hospice care to patients and families in all settings.
QUALIFICATIONS:
Educational/Degree: Graduate from an accredited school of nursing.
Training/Licensure: Currently licensed as a registered nurse to practice in the state.
Knowledge/Skills/Ability: Ability to work independently, make accurate and timely judgments, supervise others, and respond appropriately to crises outside of a hospital setting. Acceptance and adaptability to different social, racial, cultural, and religious modes.
Experience: Minimum 2 years of experience as a registered nurse, preferred. Active patient contact within the past three years is preferred.
Essential Functions:
1. Develops standards ensuring safe and therapeutically effective service to patients and families, with joint responsibility with the Administrator for meeting these standards.
2. Develops objectives for Hospice and ensures their implementation.
3. Consults with the Administrator to determine staffing patterns that accomplish stated objectives and promote maximum personnel utilization.
4. Responsible for recruiting, hiring, evaluating, and terminating service personnel.
5. Plans and arranges for the orientation of new employees and conducts selected orientation classes.
6. Prepares and maintains current policies and procedures to meet Medicare, Medicaid, accreditation standards, and state and federal laws; implements and revises as needed.
7. Supports policies and procedures and uses appropriate channels for changes; establishes criteria and procedures for selection, promotion, and termination of service personnel.
8. Participates in establishing functions and qualifications for each service position; coordinates interdisciplinary team services, assuring ongoing assessment of patient/family needs and implementation of the interdisciplinary group plan of care.
9. Provides individual or group support concerning job-related stress or issues.
10. Organizes Hospice to delineate and delegate authority, functional responsibility, lines of relationship, and communication to provide safe service.
11. Coordinates Hospice services with other community agencies.
12. Serves as a role model for colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.
13. Observes confidentiality and safeguards all patient-related information.
14. Accepts responsibility for regular attendance and punctuality, fulfilling job requirements without regard to time involved.
15. Serves as a resource person to employees.
16. Develops a cooperative relationship and communicates effectively and professionally with physicians.
17. Investigates and reports any problems relating to patient care or conditions that might harm patient and/or employee well-being.
18. Immediately reports any accidents, incidents, lost articles, or unusual occurrences to the Administrator.
19. Attends pertinent continuing education programs and shares information with staff.
20. Works with the Administrator in identifying budgetary requirements and determining appropriate use of allocations.
21. Monitors equipment for appropriate use and minimizes misuse.
22. Oversees Hospice's ongoing Quality Assessment/Performance Improvement (QAPI) program.
23. Provides 24-hour on-call coverage.
24. Plans and supervises the Hospice.
25. Ensures that patients’ plans of care are developed, implemented, and evaluated.
26. Conducts patient case conferences, in-services, staff meetings, and maintains documentation; participates in community education projects.
27. Ensures availability of all necessary supplies and equipment.
28. Remains available at all times during and after operating hours; may designate a qualified temporary replacement if unavailable.
29. Ensures compliance with federal, state, and local rules, accreditation standards, and Hospice policies in all patient care aspects.
30. Oversees staffing and assignments.
31. Addresses and corrects all patient concerns.
32. Other duties as assigned.
