The Position: We are looking for a Case Manager, who is willing to embrace McKinley’s mission and H.U.M.A.N values. The role of the Case Manager is to work in concert with the treatment team and particularly the STRTP Clinical Social Worker to liaison with the residents’ families and county workers with regards to the progress and treatment issues of the individual clients. The Case Manager will assist in coordinating the Needs and Service Plan (NSP) and treatment/case conferences.
Key Responsibilities:
- Maintain a caseload of clients and follow them from admission to discharge.
- Complete appropriate documentation, assist with the implementation of clinical interventions, and participate in an interdisciplinary treatment team for the clients on their caseload.
- Facilitate psycho-educational groups for the clients at the facility, when indicated.
- Contribute to the implementation of client behavior plans/appropriate interventions established by the Clinical Team, engage in crisis intervention when required, and ensure consistently high quality of client care to an at-risk youth population.
- Implement case plans through good professional judgment while maintaining a team approach.
- Engage in crisis intervention and client de-escalation as necessary.
- Liaison with the residents’ families and county workers regarding the progress and treatment issues of individual clients.
- Assist in coordinating the Needs and Service Plan (NSP) and treatment/case conferences.
- Other related duties as assigned.
Qualifications:
- Bachelor’s Degree (OR) Associate’s degree plus four years of experience.
- DMH experience preferred.
- Residential experience preferred.
- Proficient in Microsoft Office Suite (MS Word, Excel, PowerPoint, Outlook) and other office-related software.
- Possess superior oral and written communication skills as well as interpersonal skills.
- Detail oriented and organized.
- Able to work independently and organize multiple projects with minimum supervision.
- Able to work as a team member.
- Able to maintain the highest level of confidentiality and professional boundaries with clients and staff.
- Able to represent the agency professionally and appropriately with all constituencies.
- Must pass pre-employment physical exam, TB and drug screening.
- Ability to commute to various sites on and off campus.
- Ability to lift 10 pounds for filing.
- Must complete crisis prevention/intervention training (Pro-ACT) within the first 90 days of employment.
Compensation and Benefits:
