Job Title: Assistant Program Manager - Connecticut

FLSA Status: Non-exempt
Summary/Overall Responsibility: The Assistant PM, in conjunction with the PM, is responsible for the individual care and instruction of the individuals living in a KenCrest community home, in accordance with state and Agency guidelines. The supports are based on identified needs and outcomes resulted from frequent service planning activities. This employee is responsible for maintaining a healthy emotional and physical environment for the residents and ensuring that the home reflects the needs and individuality of the residents, in a timely fashion. This includes other responsibilities and duties as assigned.
Key Relationships:
- Receives supervision and support from Program Managers, Program Coordinator, Assistant Director, and Director. Has key relationships with direct support staff, trainers, consumers, and families.
- External stakeholders such as day programs, community contacts, and state personnel.
Essential Functions:
Coordination of Services and Support:
- Complete forms and reports as directed by the PM, including entries in the daily log book. Ensure staff are reading and signing the Communication log before each shift.
- Attend and participate in IP Planning Meetings for the individuals as directed by PM.
- Attend and participate in other required meetings, including team meetings and staff meetings as directed.
- Schedule, track, and attend medical appointments for all individuals, and complete required paperwork.
- Conduct daily medication pass checks, weekly Kardex audits, and monthly medical cabinet audits.
- Order medications as needed.
Advocacy/Direct Support Responsibilities:
- Model and teach the principles of Positive Approaches, Personal Outcomes of individuals, and empowerment techniques.
- Assist individuals in making informed choices, trying new experiences, developing relationships, and becoming self-advocates.
- Assist individuals with household management, including shopping, meal preparation, cleaning, laundry, and all other household chores.
- Provide well-balanced, nutritious meals, following any established diet plans.
- Assist individuals with personal hygiene care, including bathing and toileting individuals who need assistance with these tasks.
- Transport individuals to appointments, jobs, day programs, and any other community outings, adhering to all vehicle and driving procedures and regulations.
Supervision:
- Assist the PM with the supervision of the home, including the supervision of staff. Has a clear understanding of KenCrest Policy and Procedure and holds staff accountable.
- Responsible for the staff schedule within budget limits and ensuring proper staff ratios.
- Assume responsibility for the supervision of the home in the absence of the PM.
- Carry out goal plans and behavior management programs, and record data as required.
- Assist PM with feedback for 90-day probations and evaluations.
- Provide instruction in self-help skills, social skills, and community orientation. Know and communicate updated client information and safety procedures.
Agency Responsibilities:
- Administer medications to individuals accurately and in accordance with policies and procedures, check month medications/mars, and monitor daily administration and documentation.
- Communicate effectively and professionally with individuals, co-workers, families, and community members.
- Maintain awareness of health of individuals, including monitoring for signs and symptoms of illness or other problems. Has a thorough understanding of medications, behavior plans, and updated IPs.
- Complete all required trainings.
- Carry out/ensure completion of fire drill by the 15th of each month, ensure for a rotation of shifts.
Minimum Qualifications:
- One (1) year related experience.
- High school diploma or GED.
- Valid driver’s license.
- Ability to pass all required trainings, including CPR, and First Aid.
- Pass and maintain certification in Medication Administration.

- Ability to work proactively within a team, both at the work site and with other KenCrest staff, professionals, and members of the community.
- Ability to communicate effectively, both in oral and written form.
- Demonstrated respect for the rights and confidentiality of the individuals.
- Ability to professionally represent KenCrest Services’ mission and philosophy.
- Performs at the Second Level (Guide and Assist) in the KenCrest core competencies of communication, financial responsibility, follow through, innovation, integrity and trust, job skills/functional knowledge, leadership, positive approaches/respect for others, problem solving, responsiveness, safeguards, and team participation, as measured by the Talent Reservoir evaluation tool.
Physical Demands:
- The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Be capable of using one’s body in a variety of postures and positions for varied periods of time (i.e., kneeling, crouching, sitting, standing, reaching, climbing/descending stairs) while working; be able to lift and move objects up to 50 pounds and do medium physical work; be able to push/pull and maneuver objects or equipment required for work. Ability to lift up to 75 pounds when performing a two-person lift/transfer of an individual. Ability to drive up to 50 miles.
- Be capable of visual and auditory scanning of the work environment so as to monitor the facility to ensure safe and effective operations.
Working Conditions:
- Staff in this position work in a typical home setting in the community. Noise level is moderate. The work schedule may require flexibility, including the ability to remain at work until relieved and/or to work overtime hours as required. Some weekend and holiday shifts are typically required.
