Nurse Practitioner/ Physician Assistant-Home and Supportive Care
Responsible for providing clinical care both in the inpatient and outpatient settings to patients referred to outpatient Palliative Care. The Nurse Practitioner will utilize advanced clinical skills and knowledge to assess patients and manage all aspects of Palliative care, from a holistic perspective. This Nurse Practitioner will guide care for those patients with chronic and life limiting illness including Cancer at any stage, and other chronic illnesses. In an interdisciplinary manner, the Nurse Practitioner will provide symptom management to promote comfort/quality of life at any stage of disease process in addition to having goals of care discussions with patients and families to ensure their treatment choices reflect their wishes. The Nurse Practitioner will also collaborate closely with various other health care team members including physical/occupational therapists, dietitians, speech therapists, and case managers to deliver optimal palliative care addressing patients’ and families’ physical, social, spiritual, and emotional needs.
Education and Experience
Minimum Required Education: Must be a Registered Nurse licensed in the State of Virginia or compact state. Successful completion of an accredited Nurse Practitioner Program.
Preferred Education: MSN
Experience: Prefer minimum of 3 years’ experience working in an outpatient, adult clinic setting. Knowledge of community resources a plus.
Certificates, Licenses, Registrations
Applicant must be licensed to work in the Commonwealth of Virginia as a Nurse Practitioner, and must have current ACLS and AANP/ANCC certifications.
