Position Summary:
The incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. Assists with procedures, patient education, outpatient and post-hospital discharge follow-up including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse incorporates the psycho-motor and age-appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention, and evaluation.
Salary Range:
Hourly Rate Minimum $59.93 Midpoint $71.10 Maximum $82.27
Required Qualifications:
- Ability to establish and maintain effective working relationships across the Health System
- Ability to maintain a work pace appropriate to the workload
- Must demonstrate customer service skills appropriate to the job
- All external candidates must have a Bachelor of Science in Nursing
- Minimum of 6 months demonstrated nursing experience in an acute care or ambulatory care setting
- Ability to read, write and communicate effectively in English
- Ability to work variable shifts including evenings, nights, weekends, and holidays
- Ability to maintain flexibility and work well in a constantly changing environment
- Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Licenses and Certifications:
Basic Life Support (Required)
Preferred Qualifications:
- Understanding of applicable regulatory requirements
- Cardiovascular clinical experience
- Bilingual skills to communicate effectively with patients and families
