The Clinical Director is responsible for overseeing and directing the patient care operations of the home care facility. This includes coordinating and completing assigned projects to effectively support the immediate and long range objectives of the company; oversight of the eligibility of patients referred to home care services, planning for the services to be provided to patients and supervising their total home health care; implementing and maintaining administrative practices, agency philosophy, goals, and policies which assure compliance with applicable state and federal regulations; enhancing the profitability of the agency; and providing motivation and retention of a qualified staff and assuring the quality of services delivered. This position also acts as a liaison with management staff and other departments throughout the company.
Key Responsibilities:
- Supervises and ensures high quality patient care services throughout the episode of care, including overseeing referral coordination, continual assessment of patient needs, and development and implementation of individualized plans of care.
- Ensures safe, economical, and efficient patient care adhering to relevant standards, including federal and state regulations, and Joint Commission standards.
- Monitors and assesses agency staff performance regularly using the clinician scorecard and yearly performance evaluation tool.
- Ensures hiring of qualified personnel, overseeing licensure verification, certification, and credentialing as required by law and policy.
- Assists with staff recruitment to maintain adequate staffing levels within the staffing model.
- Serves as a member of the Management Team and QAPI Team, implementing Performance Improvement Projects and annual strategic agency planning.
- Conducts monthly meetings with the medical director, approves medical director invoices, and submits them for timely payment.
- Drives operational metrics, ensuring the organization meets goals related to efficiency, growth, quality, and finances.
- Acts as Emergency Coordinator in the absence of the Executive Director/Alternate Executive Director during emergencies.
- Communicates effectively with all customers to represent the agency and ensures thorough investigation of concerns voiced by patients, caregivers, and referral sources.
- Performs all other duties as assigned.
License Requirements:
- If Clinical Director is an RN, must have current RN licensure in state of practice.
- Current CPR certification required.
- If Clinical Director is in another clinical discipline (PT/OT/SLP), must have current applicable license in the state.
- One year of supervisory and/or management experience in a home health setting or related healthcare setting preferred.
- Current Driver's License and vehicle insurance, and access to a dependable vehicle or public transportation.
All CMS and state regulations must be followed regarding supervision of nursing services.
If Clinical Director is not an RN, the Executive Director/Administrator or Patient Care Manager must be designated as the supervising RN for nursing services unless state regulations dictate otherwise.
