The Palliative Care, Care Manager MSW works collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness.

Shift Details: Full-time, 40 hours weekly, Day shift
Required Qualifications:
- Master's Degree - In Social Work or related field from an accredited educational institution.
Preferred Qualifications:
- 1 year - Recent inpatient clinical experience in an acute care facility.
- Case management experience.
- Health care utilization review, discharge planning, and/or social work experience.
- Psychiatric based experience.
