Easterseals NH services include early childhood centers of excellence, medical rehabilitation, camping and recreation, vocational services, senior services, substance use treatment, adult day programs, community-based services, individual service options, transportation services, residential service options, and veterans services.
We are searching for a Senior Community Health Nurse to join our team and offering a $5,000 sign-on bonus!
Responsibilities:
- Perform an initial assessment of the client by providing a nursing diagnosis.
- Design and implement a care plan evaluating the outcomes.
- Provide for the emotional, physical and spiritual needs of clients.
- Administer medications, to include parenteral therapy, report pertinent clinical assessment findings to the physician, and collaborate with physicians and other disciplines in case management.
- Record client data utilizing computerized documentation systems in a timely manner per Agency policy.
- Collaborate with Intake Staff for client needs in the home.
- Conduct ongoing training of home health aides as needs arise.
- Assist with ongoing Agency health status monitoring and documentation for federal and/or state regulatory requirements.
- Participate in quality assurance/utilization review activities on a monthly basis.
- Develop and grow the nursing program in designated geographic areas.
- Facilitate all LNA and PCSP evaluations.
- Provide supervision of LPN in client care activities when necessary.
Qualifications:
- Current RN registered nurse license with the New Hampshire Board of Nursing required.
- Five (5) years of experience working in home care.
- Three (3) years of supervisory experience.
- Valid driver’s license and reliable transportation.
Location: Candidate must be able to travel between Manchester, Nashua, Stratham and Rochester, NH locations.
Compensation: $80,000.00 salaried, PLUS a $5,000.00 Sign on bonus!
Bonus will be paid in two payments and bonus amount is based on hours at time of hire as reflected in new hire offer letter.
