Physical Therapist (PT) Per Diem
We are seeking a dedicated and compassionate Physical Therapist to join our home health care team. This per diem role is ideal for candidates passionate about improving clients' quality of life in their own homes. As a key member of our interdisciplinary healthcare team, you will use your expertise to evaluate and treat residents, ensuring their progressive mobility and optimizing function across various conditions. This position demands a dynamic, proactive professional keen on contributing to individualized care plans and achieving consequential outcomes both in patient care and therapeutic success. Your role will also extend to offering education on home-based exercises and interventions that benefit both patients and their families.

Essential Duties and Responsibilities:
- Review and evaluate patient data to comprehensively assess and tailor therapy interventions.
- Execute detailed functional assessments utilizing standardized procedures and craft personalized treatment plans with achievable functional goals.
- Ensure adherence to treatment guidelines and maintain meticulous documentation for therapy sessions and outcomes.
- Provide instruction and supervision to Assistants in therapy procedures, ensuring efficient and safe execution.
- Communicate effectively with other healthcare professionals to facilitate optimal patient care and manage extensive follow-ups as necessary.
- Initiate and manage detailed discharge plans that clearly delineate therapy results and future healthcare recommendations.
- Design and implement patient and family education on post-discharge exercises and therapeutic practices to enhance recovery and manage expectations.
- Actively participate in interdisciplinary team conferences discussing patient progress, addressing potential issues, and adjusting care plans accordingly.
- Stay current with continuing education, participate in community outreach, and adhere to all regulatory and professional standards.
- Engage in quality improvement processes, demonstrating a commitment to professional growth and excellence in physical therapy.
Minimum Position Qualifications:
- Bachelor/Masters of Science degree in Physical Therapy.
- Current licensure in the state of practice.
- Proven ability to develop and execute patient-centric treatment plans.
- Strong knowledge of accreditation standards and compliance requirements.
- Physical ability to perform job-related duties, which may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing, and pulling.
- A detailed understanding of physical therapy modalities and equipment usage.
Preferred Qualifications:
- Previous experience in a home health setting.
- Familiarity with electronic health record systems.
- Certified in specialty areas of physical therapy, such as neurology, orthopedics, or geriatrics.
- Experience supervising therapy assistants and aides.
- Demonstrated ability to work independently with robust decision-making skills.
