Position Summary: Active LPN License needed - Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers.
Remote/Hybrid role: 32 Hours per week; 4-5 evenings per week 3-9pm, with weekend requirement and rotating holidays.
Experience:
- LPN with Minimum 2 years related experience including medical/surgical and community health and/or experience with a managed care organization.
- Previous intake, liaison or discharge planning experience is preferred.
- Knowledge of home care standards and practices required.
- Knowledge of medical terminology and treatment modalities of patient diagnoses required.
