Position: Full-time Home Care Registered Nurse
Schedule: Day shift, 3-4 assessments per day.
Rate: $65/hr. BONUS if eligible, $5,000 at 3 and 6 months.
Job Description:
Legacy Care Group is partnering with Commonwealth Care Alliance and is seeking compassionate and skilled Registered Nurses to assist with MDs and Functional Assessments for Enrollee's in central Massachusetts. This role has a focus on community-health and providing support to individuals in under-served populations. Your experience and expertise can contribute to improving healthcare outcomes for our communities.
Key Responsibilities:
- Comprehensive MDS and Functional Assessments:
- Evaluate the Enrollee's living conditions, safety, medical history, psychiatric and substance abuse history, functional abilities, medication regimen, social history and other factors that may impact determination of their medical, behavioral and long-term support service needs and the urgency of such needs
- Determine the Enrollee's One Care Plan rating category
- Enrollee education regarding the determined Care Plan
- Complete a reassessment whenever there is a significant change in health or behavioral health status as determined by the Care Manager, when there is evidence that the Enrollee's One Care Plan rating category might have changed; and prior to the annual assessment.
- Provide compassionate care and emotional support to patients, employing effective communication strategies tailored to individual needs.
- Utilize nursing assessment skills to identify and address complex health issues among diverse patient populations, including those with histories of substance abuse or mental illness.
- Implement evidence-based practices and harm reduction techniques to mitigate health risks and promote overall well-being.
- Collaborate with interdisciplinary teams and community resources as needed.
- Advocate for patients' rights and address systemic barriers to healthcare access and equity.
Job Requirements
Education:
- Graduate of an accredited nursing program. Bachelor of Science Degree in Nursing preferred.
Licenses & Certificates:
- Massachusetts licensure as a registered nurse required.
- Basic Life Support (BLS) certification required.
- Valid driver's license and insured personal vehicle, or other reliable means of transportation
Experience:
- Minimum of 2 years of nursing experience, preferably with home care experience.
- Experience working with Mass Health beneficiaries and individuals with psychiatric and/or substance abuse conditions is preferred.
- Good knowledge of using the RFS and MDS-HC, functional assessment tools
- Ability to write an individualized care plan and communicate effectively, verbally and in writing, across complicated service
and support systems;
Skills:
- Strong nursing assessment skills, particularly with diverse populations such as individuals experiencing homelessness, those with histories of substance abuse or mental illness.
- Ability to conducting comprehensive initial assessments, encompassing social, medical, and mental health evaluations.
- Highly organized and time-efficient.
- Strong documentation skills, completed in a timely manner.
- Strong communication and interpersonal skills.
- Proficiency in multiple languages is highly desirable.
