Job Description - Nurse Practitioner-Family Medicine-Physician Practice Full Time (3310489)
PURPOSE OF YOUR JOB DESCRIPTION:
The purpose is to support the Family Medicine Physician Practice and to provide coverage when the providers are out of the office.
DELEGATION OF AUTHORITY:
They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well.

PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIES:
- Diagnose and Treat acute and chronic Illness
- Perform age-appropriate history and physical
- Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
- Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age-specific need
- Monitor the effectiveness of treatments
- Provide education of acute and chronic medical conditions
- Managing Chronic Medical Conditions
- Reconciling medication list
- Facilitating appointments to specialist
- Providing thorough exams and evaluations of patient’s health status
- Facilitate appropriate chronic care management (i.e. Diabetic eye exams, foot exams and nutritional counseling)
Preventive Medicine:
- Keeping track of and referring patient to appropriate preventive medical screenings (e.g. Mammograms and Colonoscopies)
- Counseling and reinforcing patients in healthy life style habits
EDUCATION REQUIREMENTS:
• Primary Care NP certified in Massachusetts
EXPERIENCE:
• 3 years experience in a Primary Care setting
• Proficient Computer Skills (MS Word)
• EMR experience required
• Ability to document in electronic health record completely and accurately
• Knowledge of clinical reminders and how to use them appropriately
• Good communication skills, both spoken and written
• Willing and able to work in a team environment
• Able to change with the ever evolving healthcare field
• Master Degree, ACLS, and PALS preferred
