Next Care Hospice is a fast-growing hospice agency that provides compassionate care to our patients and their families during the end-of-life journey.

We are currently in need of an experienced Per-Diem Hospice RN in the Murietta/Temecula area to provide skilled nursing to hospice patients at home and senior facilities. A minimum of one year of experience is required. The RN must have experience in hospice care and be able to complete each visit with thorough recording of notes in the EMR system (Hospice MD).
Our current per-visit rates are as follows:
- Supervisory Visits: $75/visit
- Re-Certifications: $100/visit
- New Admissions: $250/admission
We are also hiring salaried full-time or part-time RN Case Managers.
Our hospice patients are located in Los Angeles County, Orange County, and surrounding areas. The RN is expected to conduct supervisory visits, recertifications, and new admissions.
You will be responsible for the assessment, diagnosis, and treatment of assigned patients.
The ideal candidate will be a responsible and well-trained professional able to provide the best nursing care with minimal supervision. You will be compassionate and cool-tempered, able to follow health and safety guidelines faithfully and consistently. You will initiate and coordinate the hospice care plan, supervise CHHAs and LVNs, and provide skilled nursing hospice and palliative care services to our patients. The RN will work cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN regularly reviews and updates the plan of care, performs prescribed medical treatments (including pain management and symptom control), conducts assessments and evaluations, and provides education and supportive care to patients and caregivers. Must be comfortable utilizing computers and EMR (Hospice MD). Hospice MD training can be provided.
QUALIFICATIONS: Current RN license within the State. Hospice experience required. Knowledge of EMR software.
Pay: $75.00 - $250.00 per hour
