General Description
Promotes the physical and mental health of those supported by applying nursing practices, as achieved as a member of the interdisciplinary treatment team with the goal of assisting people supported to assume responsibility for their own health. The nurse coordinates and monitors medical services, including psychiatric and medication services, in accordance with the requirements of regulatory agencies and in response to individual needs.

Essential Functions are the minimum requirements of the position
- Participates in the admission of new residents to assigned homes.
- Clarifies initial or hospital discharge medication orders and contacts the Primary Care Physician or Psychiatrist to review the status and medication plans.
- Develops plans to ensure that the medical and health care needs of residents are met. This includes:
- Coordinating the annual physical examination
- Ensuring appropriate medical follow-up
- Providing special instructions to resident and home staff related to medical diagnosis, allergies, medication, diet, special emergency plans related to ongoing medical conditions.
- Coordinating and reviewing lab work
- Coordinating care with clinics, physicians and hospitals
- Health teaching for all employees
- Functions as part of an interdisciplinary team, attending staff and team meetings, Individual Plans (IP), six month, quarterly and 30-day reviews.
- Is available for ongoing consultation to the team regarding medical problems and health care needs.
- Completes the medical sections of the IP's, six month, quarterly and thirty-day reviews that address health care needs and issues.
- Documents resident's services in accordance with Agency policy and RI state regulations.
- Maintains records including: monthly nursing reports, plans of care, monthly medication charts and book, incident reports, medical history, hospital intake/discharge, immunization history, nursing review.
- Reviews medicine closet/cabinet periodically.
- Promotes resident empowerment and self-care activities including self-medication.
- Visits residences twice per week while the individuals are home.
- Maintains communication with workshop, school, family and professional resources.
Non-Essential Functions
- Covers for peers during vacations and other times due to business needs.
- Participates in Agency training activities.
- Teaches and supports staff on signs and symptoms of illness, vital signs and medication management.
- Participates in the orientation of new nursing staff and team members.
- Participates in community and other public education regarding mission and philosophy.
- Attends staff, discipline and committee meetings as assigned by Supervision.
- Submit monthly work schedule and mileage vouchers, and vacation requests to the Assistant Director.
- Reviews and signs incident reports and provides follow-up.
- Assumes responsibility for professional development.
- All other duties as assigned by supervision.

Education and/or Experience
Graduate of an accredited school for nursing. Bachelor of Science or Associates Degree in Nursing. Minimum of two years of experience working as a registered nurse including some medical/surgical experience and awareness of the needs of the chronically and mentally ill.
Certificates, Licenses, Registrations
State of Rhode Island License as a Registered Nurse.
