Hospice Case Manager – Cobb County
We have recently engaged with a very progressive, well-run, and financially stable Hospice organization in the Cobb County area of Georgia. They are a nurse-owned and operated organization that is committed to patient satisfaction, clinical excellence, and quality care for each member of the family.
Our client is offering the following benefits:
- Annual Salary: $80,000-$90,000 +
- Health Insurance - Employer Pays % COBRA
- Dental and Vision Insurance - Employer Pays % COBRA
- Mileage Reimbursement - $0.48 Per Mile
- PTO - 14 Days
- Paid Holidays - 7 Federal
- 401K with 3% Match
Responsibilities:
- The RN Case Manager will maintain a case load of about 12-14 patients.
- Must have MatrixCare experience.
- Patients per day - 4 on average.
- The RN Case Manager will be responsible for back-up call.
Requirements:
- Registered Nurse in the State of Georgia.
- 2-3 years of Hospice or Palliative Care experience - Preferred.
- 2 years of in-home patient care experience - Required.
- Familiarity with EMR System MatrixCare - Preferred.
