An exciting 12 month secondment opportunity has arisen for an experienced clinical pharmacist to join South Tyneside and Sunderland NHS Foundation Trust as a patient-focused Advanced Practitioner Pharmacist working in the Lipid service.
The role involves providing patient-focused advanced clinical support to the Lipid Service team, and you will work closely with the team to ensure a high quality delivery of service for patients attending the Trust. You will have the opportunity to utilise independent prescribing skills through the delivery of established pharmacist-led Lipid clinics and work with the team to develop services within this specialty.
Main Duties of the Job
You will have evidence of effective service delivery at a specialist pharmacist level, post-graduate study to diploma level, well-developed clinical pharmacy skills with an independent prescribing qualification, and evidence of advanced clinical skills development. The Pharmacy Service operates a seven-day clinical service with routine support of both junior pharmacists and integrated medicines management technicians.
South Tyneside and Sunderland NHS Foundation Trust has invested in modern pharmaceutical facilities including full electronic prescribing with decision support, automated dispensaries, and extensive seven-day clinical services to enhance the patient experience of care by improving the quality, safety, and effectiveness of their medicines utilisation. Your role will be to lead and co-ordinate the professional clinical and pharmaceutical input for this patient group with a focus on medicines optimisation, safe and secure utilisation of medicines, and minimising the risk of inappropriate use.
Job Responsibilities
- To lead and co-ordinate the professional clinical and pharmaceutical input into the safe and cost-effective utilisation of medicines in conjunction with the Trust Lipid Services and lead consultants.
- To provide expert advice in the designated area undertaking risk management, financial management, and clinical governance activities and undertake prescribing to deliver medicines optimisation and safe and effective discharge.
- To monitor performance of pharmacists working in this area, to undertake clinical supervision and internal and external teaching for all staff.
- To co-ordinate clinical service delivery in conjunction with other advanced practitioners and directorate lead pharmacists to ensure safe and efficient care from pre-assessment to discharge.
- To link with the antibiotic lead pharmacist to deliver effective antibiotic utilisation and initiation to reduce the incidence of Hospital Acquired Infections.
- To work at an advanced level in the designated areas of practice.
Person Specification
Experience
- Evidence of effective service delivery of direct patient care at specialist practice level.
- Close and effective working with medical staff.
- Leadership of audit projects and/or research.
- Clinical mentoring or Pre-registration tutor.
- Evidence of delivering successful change programmes.
- Research and evidence of publications.
- Teaching at higher education level.

Qualifications
- Postgraduate Study to Diploma level or equivalent experience.
- MRPharmS.
