Summary:
All About Home Care is a Houston-based home health care agency. We’ve been serving the Houston and surrounding communities since 2003, providing quality home health services. We are looking for a Physical Therapist (PT) to help serve patients in and around the following areas: Cypress, Humble, Channelview, Atascocita, Crosby, Baytown, Pasadena, Pearland, Sugarland, Webster, League City, Friendswood, Dickinson, Kemah, and Texas City. If you cover any of the areas mentioned above and have experience in home health, please submit your resume. We use Kinnser software to document patient care.
Qualifications:
- Graduate from a Physical Therapist curriculum approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Currently licensed as a Physical Therapist in the state of Texas.
- Two years of experience preferred.
- Ability to exercise initiative and independent judgment.
- Spanish speaking is a plus.
- Ability to complete OASIS visits and documentation (Start of Care, Re-certification, Resumption of Care).
Responsibilities:
- Makes the initial assessment visit and generates OASIS documentation the same day of the visit.
- Understands and adheres to established policies and procedures.
- Provides physician-prescribed physical therapy.
- Performs evaluation and treatment visits.
- Improves or minimizes the residual physical disabilities of the patient.
- Returns the individual to optimal and productive level within the patient’s capabilities.
- Participates with all other healthcare personnel in patient care planning.
- Directs and supervises personnel as required.
- Performs all skilled procedures as ordered by the physician.
- Consults with physicians regarding changes in treatment, reports to physicians regarding patient progress, instructs patients and families/significant others in home programs and activities of daily living.
- Participates in in-service programs and presents in-service programs as assigned.
- Participates in performance improvement activities as assigned.
- Attends all patient care conferences as scheduled.
- Prepares clinical and progress notes daily.
- Assists physicians with evaluating the level of function.
- Helps develop the plan of care and revise as necessary.
- Consults with family and agency personnel.
