Family Nurse Practitioner - Primary Care
We are seeking a dedicated and experienced Family Nurse Practitioner to join our Primary Care practices located in the beautiful Finger Lakes region of New York State, specifically in Dundee, Seneca Falls, and Penn Yan. This full-time role is focused on providing high-quality, primary health care via a direct hire position within our healthcare system. Ideal candidates will build long-term relationships with their patients, delivering consistent and comprehensive care. Compensation: $115,000-$135,000
Essential Duties and Responsibilities:
- Manage ongoing long-term primary patient care in both clinical and home settings.
- Evaluate and treat patients with chronic and acute health conditions.
- Order, perform and interpret diagnostic tests and analyze records, reports, and exam information to discuss conditions and treatments with patients and healthcare teams.
- Continuously update plans for patient care based on assessment of their health status.
- Prescribe medications following the latest guidelines and laws of the state of New York.
- Guide patients in managing their overall well-being through preventative care and healthy lifestyle choices.
- Respond to inquiries medically necessary as posed by both patients and professional peers.
Minimum Position Qualifications:
- Board Certification as a Nurse Practitioner.
- Active NYS license as a Nurse Practitioner.
- Graduate from an accredited Nurse Practitioner program.
- Proven experience as a Family Nurse Practitioner in a primary care setting.
- Exceptional communication skills, empathy, and the ability to foster trusting relationships with diverse patients.
- Ability to make decisions quickly in emergency situations.
Preferred Qualifications:
- Experience in managing both individual and population health needs.
- Confidence using electronic health records and telemedicine platforms.
- Experience working in a rural health context is an advantage.
- A strong desire to participate in community healthcare education initiatives.
- Bilingual abilities particularly in Spanish, are highly advantageous.
- Familiarity with the healthcare landscape of New York State.
