Family Nurse Practitioner - Primary Care
We are actively seeking dedicated and compassionate Family Nurse Practitioners to join our primary care practices in the scenic Finger Lakes region of New York. In this role, you will provide comprehensive and continuous primary care to a diverse patient population, focusing on prevention, wellness, and the management of chronic conditions. Compensation: $115,000.00 - $135,000.00
Essential Duties and Responsibilities:
- Conduct comprehensive assessments of patients' health including physical examinations and diagnostic tests.
- Formulate, implement, and monitor treatment plans addressing acute and chronic conditions.
- Prescribe medications and therapies based on patient-specific information.
- Educate and counsel patients and families on health, lifestyle, and disease prevention.
- Collaborate with healthcare professionals to coordinate and optimize patient care.
- Regularly review and update patient records, maintaining high standards of confidentiality and efficiency.
- Participate in ongoing professional education and development to ensure best practices in primary care.
Minimum Position Qualifications:
- Current Board Certification as a Family Nurse Practitioner.
- Active NYS License as a Nurse Practitioner.
- Proven experience as a Family Nurse Practitioner in a primary care setting.
- Strong ability to develop and maintain long-term patient relationships.
- Excellent communication and interpersonal skills, with a focus on patient-centered care.
Preferred Qualifications:
- Experience in rural community health settings.
- Advanced certification in areas such as Pediatrics, Gerontology, or Women’s Health.
- Demonstrated leadership abilities in a healthcare setting.
- Prior experience with electronic health record (EHR) systems.
- Bilingual abilities, particularly in Spanish, a plus to cater to a diverse patient base.
