About the Job:
The Advanced Registered Nurse Practitioner is responsible for providing general medical care and treatment to clients, under the direction of physician. Performs history, physical examinations, and preventive health measures on assigned clients; provides consultation and liaison throughout the facility; performs daily rounds on clients as directed by supervising physician; prescribes medications; and provides clinical assessments and diagnosis. Works collaboratively with the medical/clinical staff under the guidelines of state licensure for Nurse Practitioners.
Roles and Responsibilities:
· Gathers, develops, and evaluates age-appropriate history and physical (H&P) client data in collaboration with client, family and health care team for physician’s use in treating clients.
· Assesses, reassess physical, psychosocial and cultural needs of clients and formulates measures to meet client needs.
· Provides screening and medication management to clients served.
· Establishes priorities in planning and organizing client care based on H&P assessment. Participates in the formulation of client Master Treatment Plan
· Encourages family involvement in client care. Responds to identified client and family educational needs. Assists in client teaching as it relates to managing medical needs or medication education.
· Diagnoses and treats acute, episodic, or chronic illness, independently or as part of a healthcare team.
· Assists the physician with the clinical operation of the program
· Documents pertinent data including assessment, interventions, and evaluations. Charts clinical observation (direct or through dictation), evaluations and treatment of client in required time according to facility guidelines.
· Collaborates with physicians and other staff on establishing realistic outcome goals as needed.
· Implements appropriate health care interventions as needed, using appropriate guidelines of Family Nurse Practitioner (FNP) prescriptive practice.
· Reviews medical reports released under the name of the Facility for assigned clients.
· Provides guidance to the clinical staff working with assigned clients to assure that quality of care is provided.
· Prepares discharge summaries for physician review.
· Under physician collaborative practice agreement as a Family Nurse Practitioner with prescriptive privileges, will call permissible medication to pharmacies.
· Provides on-call responsibilities on scheduled basis directed by medical staff.
· Focuses on health promotion and disease prevention. May order, perform or interpret diagnostic tests such as lab work and x-rays. May prescribe medication.
· Master of Science of Nursing degree from an accredited school of nursing required.
· One or more years’ experience as an RN with the client population of the facility line of business required.
· Experience in prescribing basic psychotropic medications that would ordinarily be managed in a primary care setting.
· Clinical experience working with the client population of the facility; ability to demonstrate knowledge of concepts of limit setting, therapeutic boundaries, crisis management, therapeutic treatment, environment, and client care required.
· Current Nurse Practitioner license required, per state regulations/requirements, may also require a current DEA license.
· CPR and de-escalation certification required (training available upon hire and offered by facility).
· First aid may be required based on state or facility requirements.
