This position is eligible for 10% field work specialty pay for hours worked.
The first 3 months in office, then scheduled levels of telework until probation is complete.

Pierce County Human Services is the community and human services arm of the Pierce County government. It is the largest contributor to Pierce County's safety net providing services and funding to nearly 170 agencies and governmental jurisdictions. The department collaborates with its community partners, including other public and non-profit funders and service providers to understand current and emerging community and human services needs. How to be successful in the role:
Develop expertise in creating Health Action Plans and collaborating with individuals monthly to identify and monitor their health goals. Utilize person-centered interactions to engage individuals in their available services. This role will include receiving training from peers and supervision to ensure the successful achievement of position objectives.
Core Daily Responsibilities

Outreach and engagement of older and disabled individuals to improve health outcomes.
Work with eligible individuals to identify health goals and available community services to meet health needs.
Respond to individuals seeking information by providing resources, referrals and assistance.
Monitor both the progress of the individual and the adequacy of the services being provided.
Provide coordination of services and advocacy for older and disabled adult individuals.

A Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or related field AND
Two years or more experience providing direct human services, preferably in the area of providing services to older adults is required.
Possession of or ability to obtain a valid Washington State driver's license within 30 days of employment or otherwise meet the travel requirements.
Successful completion of a thorough background investigation, in accordance with RCW 43.43 is required prior to employment.
