Summary of Job Duties: The Nurse Educator will work within the identified Family Medicine Clinic to implement and promote the benefits and value of an Evidence-Based program to maintain a Patient and Family Centered Care approach. Develops a clinical strategy for Well Child Checks 0-5 years pre-visit planning and coordinates the transition of care for pediatric patients. Oversees Community Health Worker's call encounters to direct patient follow-up care. Effectively collaborates with nursing staff at all levels and participates with the clinic's complex care team to ensure children who are high-priority beneficiaries receive optimal care - bilingual preferred (Spanish, Marshallese). The Nurse Educator will support the Family Medicine Residency by providing pediatric/Prenatal knowledge and skills through daily clinical encounters and participating in residency and nursing learning activities.
Qualifications:
Minimum Qualifications:
- Graduate of an accredited Nursing Program with an associate degree or Nursing Diploma with seven (7) years of experience as an educator/instructor or RN experience OR Bachelor in Nursing and RN License plus five (5) years of experience as an educator/instructor or RN experience.
- Valid Arkansas Registered Nursing License.
- Current BLS certification or must successfully obtain certification within the first 30-days of employment. Valid AR Driver's License.
- Must be able to work evenings and weekends as needed.
Key Responsibilities:
- Promotes Screenings, Early Identification, and Clinical Navigation for Children Prenatal-5.
- Become a high-quality Early Childhood Development (ECD) specialist to ensure pediatric/Prenatal E-B practices are followed by clinical staff.
- Utilize clinical expertise in wellness developmental stages and health risk stratification to guide care management for pediatrics/prenatal patients.
- Directly supervises a Community Health Worker who provides clinical navigation, home visiting, and health education related to early childhood development under the supervision of the Program Implementation Director.
- Work cooperatively as a Family Medicine Clinic team member to ensure project initiatives and goals are met and promote health equity.
- Partners with the clinic team to coordinate and support overall health and wellness for pediatric clients and families.
- Ensures pediatric clients' screenings, assessments, and treatment planning are conducted according to chronological and developmental age.
- Coordinate referrals, support services, and treatment pathways for pediatric patients and their families.
- Submit timely and accurate documentation of services, billing data, and required paperwork.
- Support continuous quality improvement within clinics.
- Provide training and continuing education opportunities for the clinic and community on early childhood healthy development.
- Participate in ongoing professional development.
- Attend all clinic and project meetings and trainings as required.
- Exhibits competency with technical skills and EHR documentation.
- Exhibits professionalism by maintaining a positive working relationship with patients, providers, and employees.
Develops and manages relationships with program participants and community stakeholders.
- Coordinates communication with community stakeholders.
- Supports clinic's Patient Family Advisory Council by fostering P-5 Family Voices participation.
- Responsible for relationship building and management with external partners.
- Frequently tracks and reports on program deliverables through effective utilization of program management software.
- Identifies barriers and utilizes good problem-solving skills to address issues that arise regarding participant relationships, ensuring all program objectives are met, and data collection processes and procedures are followed.
Inform program and support clinic decisions as related to culturally adapting services to the community.
May perform other duties as assigned.
