RN Case Manager - Full-time - Hospice - Home Care
$33/hour - $36/hour
$7,500 Sign On Bonus!
This is a flexible and autonomous position with a weekend rotation and a holiday rotation.

The RN Case Manager supports the mission of Hospice of Lenawee by providing care to individuals and families based upon the philosophy, policies, and procedures of Hospice of Lenawee. The RN Case Manager uses the nursing process of assessment, problem identification, planning, intervention, and evaluation.
Essential Job Responsibilities of an RN Case Manager:
- Develops and maintains a caseload of patients and families.
- Provides skilled nursing care based on a compassionate and holistic approach.
- Develops a comprehensive care plan that addresses physical, emotional, sociological, and spiritual needs of the patient/family in conjunction with the patient/family and program staff.
- Advocates for pain/symptom control.
- Teaches, supervises, and counsels the patient and family regarding care concerns.
- Maintains close communication and involvement with the attending physician, home health agency, spiritual advisors and other persons that may be participating in the care.
- Performs admission of patient to hospice care when needed.
Qualifications on an RN Case Manager:
Required Knowledge, Skills, and Abilities of an RN Case Manager:
- Familiarity with community health organizations, practices, and resources.
- Familiarity with the Hospice philosophy of care for the dying and in agreement with the principles of HOL care and a commitment to it.
- Demonstrates ability to make sound clinical judgments and act upon them in a timely manner.
- Demonstrates general technical abilities with nursing functions, such as IV administration, equipment function and maintenance.
- Ability to plan, assess, implement, and evaluate nursing care for Hospice of Lenawee patient/families.
- Ability to function autonomously in a home setting.
- Demonstrates excellent customer service, both internally and externally.
- Possesses strong communication skills and the ability to interact positively and professionally with patients, families, and other team members.
- Demonstrates ability to function as a team member, collaborate with coworkers on different shifts, positions, and departments to promote teamwork and inclusion.
- Demonstrates interest, compassion, and empathy for the terminally ill and is comfortable with issues of death and dying.
- Is self-aware and asks for help when the need arises.
Education/Credentials of an RN Case Manager:
- Graduate of approved School of Nursing.
- Current Michigan Registered Nurse License.
- BSN degree, preferred.
- CPR certification, required.
Experience of an RN Case Manager:
- Two years nursing experience in hospice, home health, or oncology nursing, preferred.
- Experience with dying patients and their families, hospice experience preferred.
- Experience in health care, required.
Additional Requirements:
- Valid driver license and automobile with current insurance.
