Position Overview: We are seeking an experienced and compassionate Acute Care Nurse Practitioner to join our prestigious transplant program at Johns Hopkins Hospital. The Acute Care Nurse Practitioner will play a pivotal role in providing comprehensive care to transplant patients throughout the continuum of their transplant journey, from pre-transplant evaluation to post-operative management and long-term follow-up. Responsibilities:
- Conduct thorough pre-transplant evaluations, including medical history review, physical examinations, and diagnostic assessments, to assess transplant candidacy and identify any potential risks or contraindications.
- Collaborate with the transplant team to develop individualized care plans for transplant candidates, incorporating evidence-based guidelines, patient preferences, and interdisciplinary input.
- Provide education and counseling to patients and their families regarding the transplant process, including risks, benefits, and post-operative care instructions.
- Assist in the coordination of transplant evaluations, including scheduling tests, consultations, and procedures, to ensure timely completion of pre-transplant workup.
- Participate in the management of patients during transplant surgeries, including intraoperative support and post-operative care in the intensive care unit or step-down unit.
- Monitor and manage post-transplant care, including medication management, wound care, and monitoring for complications, to optimize patient outcomes and prevent adverse events.
- Collaborate with primary care providers, specialists, and other members of the healthcare team to coordinate ongoing care for transplant patients, addressing both transplant-related and comorbid conditions.
- Participate in transplant program quality improvement initiatives, research projects, and interdisciplinary rounds to enhance patient care delivery and outcomes.
- Maintain accurate and timely documentation of patient assessments, interventions, and outcomes in electronic health records, in compliance with regulatory standards and institutional policies.
- Demonstrate commitment to professional growth and development through participation in continuing education, certification maintenance, and professional organizations related to transplant and acute care practice.
Qualifications:
- Master's degree in Nursing from an accredited program with specialization as an Acute Care Nurse Practitioner.
- Current licensure as a Registered Nurse and Nurse Practitioner in the state of Maryland.
- Certification as an Acute Care Nurse Practitioner by an accredited certifying body (e.g., AACN, ANCC).
- Minimum of two years of clinical experience as a Nurse Practitioner in an acute care setting, with experience in transplant or critical care preferred.
- Strong clinical assessment and critical thinking skills, with the ability to manage complex medical conditions and acute care needs.
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse healthcare teams and engage with patients and families.
- Knowledge of transplant medicine, including pre-transplant evaluation, surgical techniques, immunosuppressive therapies, and post-transplant care protocols.
- Ability to thrive in a fast-paced and dynamic work environment, demonstrating flexibility, adaptability, and resilience in the face of challenges.
- Commitment to patient-centered care, with a focus on compassion, empathy, and respect for patients' rights and dignity.
- Willingness to participate in flexible scheduling, including on-call responsibilities and weekend/holiday coverage as needed.
