Summary:
Provides pharmaceutical care according to professional standards and practices and complies with all Federal and State regulations in the practice of Pharmacy at Virtua. Clinical pharmacy services to include rounding with the Virtua Hospital at Home team and providing direct patient care to Care After Discharge outpatients. Additional responsibilities to include providing education to providers and nurses in addition to serving as a resource for drug information; developing system-wide guidelines and protocol standardization, education for pharmacy department and clinical departments as needed. Participation in system-wide committees such as Pharmacy & Therapeutics Committee, in addition to precepting pharmacy residents and pharmacy students.
Position Responsibilities:
- Participates in patient rounds with appropriate individuals including physicians, nurses and other health care providers.
- Evaluates medication prescribing patterns and identifies opportunities for improving drug therapy.
- Conducts Drug Use Evaluations.
- Provides education to patients/families, and other health care practitioners.
- Participates in medication safety initiatives.
- Works within key sub-specialty areas to develop and maintain specific routine monitoring criteria for these patients.
- Represents the Pharmacy Dept. in system wide and divisional committees as needed.
Required Education:
Graduate of an accredited College of Pharmacy with Doctor of Pharmacy (PharmD) degree or BS and comparable clinical experience. PGY-1 Required; PGY2 in Internal Medicine or Ambulatory Care Preferred.
Training/Certifications/Licensure:
Active New Jersey pharmacy license.
